Please remember your child must be under the age of 18 on the day of the draw, (proof of age may be requested) and have a social insurance number to register the plan.  Forms to attain a social insurance number for your child are available at most Banks, Service Canada, or online at www.servicecanada.gc.ca./home Push the search button and type in nas2120-04-05.
